How does the Family Services Division determine disability?

A Medical Review Team (MRT) assesses an individual’s medical condition by reviewing their medical records.

If you are applying based on a disability, do not currently receive Supplemental Security Income (SSI) or Social Security Disability Insurance (SSDI), and are 64 years old or younger, additional information will be required which includes the following:

  • A medical release form which allows MRT to request your medical records.
  • A disability questionnaire which helps MRT to know which medical records to request.
  • A list of doctors you have seen for treatment.
  • Social Information Summary.

A packet containing these forms will be sent to you.
